Thank you for registering for the Mt. Lebanon Aqua Club 2008-2009 Season
This registration and the First Payment listed below is due on or before September 17, 2008. The steps required are outlined below. A checklist is provided at the bottom of the page to ensure that you have done everything required to enable your swimmer(s) to be signed up. Incomplete registrations will be returned, so please ensure that all required forms and payments are included. All forms are linked in Step 2. Click on the link to download and print these forms. - MLAC utilizes an online registration form. Click Here to be taken to the MLAC registration form. You will be asked to fill in your information (one form per swimmer) and submit it electronically. After completing the form, you will be returned to this page where you can register a second child and finish the registration process. Information will be stored on a secure server and removed as we transfer this information to Team Manager, the Club's primary database. This information will be accessible only to registration personnel. Contact and medical information will be provided to your swimmer(s) coach.
- Click Here to download the Parent's Release and Consent form. Print this form, fill in your swimmer's name, sign, and date these forms. Please note that this form is required for each swimmer.

Payment for multiple swimmers may be made by one check. No cash please. Your remaining balance will be calculated, applying all fundraising and family work credits, and invoiced for equal payments November 1st and February 1st. You may prepay the entire balance with the November 1st payment.

For New Swimmers Only
MLAC requires that each swimmer maintain a balance of $50.00 in a Meet Entry Account. These funds are to be used for payment of fees when swimmers are entered in meets. Click Here for additional information on Meet Entry Fees. Please note that these are separate from Dues and Fees discussed above. New swimmers should remit a SEPARATE check for $50.00 made out to MLAC at the time of registration.

You can submit your payment and forms in one of three ways:
Bring them to the picnic at the Mt. Lebanon pool/park on September 7
Mail the forms and payment to:
MLAC - Attention REGISTRATION P.O. Box 13196 Pittsburgh, PA 15243Or put them in the Registration Box in the cage at the High School Pool. There is a locked box inside the cage. Please do not put these in family folders, and please, no cash.
